This is a patient administered site in which you will input as much information as you can recall about any past experiences with psychiatric medications. You may wish to request medication records from your past doctors or pharmacists to help with the processes. You can record each medication by name and input as much information as you can remember regarding length of treatment, response, side effects, and condition for which the medication was prescribed.

You can look up medications by brand or generic name, picture, description, or conditions for which the medication is typically prescribed. Pull down menus will help with dosage strengths as well.

When you have completed inputting the information, you can opt to input your doctor's FAX number or email address or look up by physician's name if he or she is registered on our site.

By having this information, you will make your doctor’s job considerably easier in helping to map out a more appropriate course of treatment for your condition. This will also save you from unnecessary repetition of unsuccessful medication trials.

How does it work?

  • Once completed, the information can be printed off for your doctor or yourself.
  • There is also an option to FAX or email it to your physician.
  • There is no charge to you or your doctor for this service.
  • You do not need to use your name to insure confidentiality.
  • In order to protect your privacy we will not store any personal data about you.
  • But be cautious, for the same reason you might not be able to retrieve your history at a later date.
  • Your information can only be stored, retrieved or modified later if you complete our free registration page and create a password.
